Background: Appendicitis is a common but perplexing medical mystery. It occurs unexpectedly and requires rapid medical intervention to be resolved. Fishbone-induced appendicitis in children is a rare but serious condition, often misdiagnosed due to nonspecific symptoms. Often, during the treatment of such patients, the fishbone has been seen in the gastrointestinal tract. However, in some rare cases, the fishbone goes into the appendix and becomes the cause of appendicitis pain. Case Report: The current report explains the case of appendicitis in a 5-year-old boy. This boy was admitted to the hospital with abnormal abdominal pain, accompanied by fever, nausea, and vomiting. Later on, the pain was identified as appendicitis pain caused by a fishbone. The patient underwent a successful laparoscopic appendectomy without signs of perforation. Conclusion: Identifying appendicitis caused by a fishbone in pediatric patients can be challenging as most parents fail to recall when their child consumed the fish. This case underscores the need for detailed clinical assessments to uncover uncommon causes of appendicitis. It is recommended to conduct thorough examinations to evaluate the possibility of intestinal perforation caused by foreign objects such as fishbones and to educate the parents to avoid serving fish with bones to their children.
